It is important we have up-to-date information on everyone who attends Fellowship Kids or Fellowship Youth. This will help us know how to best care for your child and keep them safe.

Please take a few minutes to fill in YOUR details first.

By clicking the button and submitting this form, I understand and assume all of the risks which may be encountered by my child in participating in the activities of the Kids/Youth Ministry of Fellowship Dubai. I do for myself and for my child hereby irrevocably and unconditionally release and forever discharge Fellowship Dubai and its employees, agents, and volunteers from any and all liability, actions, expenses, obligations, damages, and claims of whatever nature and which I now have or which may arise in the future, in connection with my child’s participation in activities of the Kids/Youth Ministry of Fellowship Dubai including, without limitation, injury to my child or property even injury resulting in death.

If I cannot be reached within a reasonable period of time, as determined by church employees or volunteers, I hereby authorize them to make emergency medical decisions for my child.
